Hi Jim: For 1926 death records go to the Ohio Historical Society web-site: http://www.ohiohistory.org They have an online search engine for the death records 1911-1937. (They have been scanned and some of the names are jumbled, so initially, try to do as wide a search as possible.) Once you have the information, you can order the certificate from them. And yes, the certificate should have the father's name and mother's maiden name, if it was known by the informant who gave the information at the time of the person's death.
