I'd like to get a copy of my husband's grandfather's death certificate. He died in Medina County, March 9, 1965. Do I send the 'Request for Certified Copy' form to the Ohio Department of Health or do I send something to Medina Probate Court? I'm just looking for an uncertified copy. Also, his obituary states that the coroner would rule on the cause of death. Are coroner's reports open records? Would I be able to get a copy of the report, and, if so, how would I get a copy?
