    3. I've actually done three. I too wrote them myself because to make them the least bit interesting, the story needs to be told along with the facts and documents. I used word but pdf is pretty much the standard format used by the publishers. I used a print on demand company so that I didn't have to order a 100 or more. It was time consuming but well worth it.
