I hope this works! I copied this from the Family History Library Catalog. My local one has a copy of this microfiche that does not return to Salt Lake City. It is not an all inclusive index (what is?) but it did yield a lot of information and surprises for me. Also it is a hardcover book that may be available in a library near folks in Ohio.
