Death certificates from 1945 to the present are held at the Ohio Dept. of Health (Vital Statistics Division). Death certificates from December 1908 - December 1944 are held at the Ohio Historical Society. Even if the microfilm at OHS if bad, if you can provide the volume and certificate number, the name of deceased, date and place of death (all listed on the index), they will make a copy from the original death certificate. County and local health departments also have death certificates on file from December 1908 to present. Deaths before December 1908 will be found in the probate court of the county where the death occurred.
