They were $32.00 a few months ago and the "next of kin" has to get it. I do know you need a certified death certificate also. When I went downtown they gave me forms that needed to be filled out. I will look for them in a couple days (I'll going out of state tomorrow), and I'll let you know what they are. There are two children alive and I have to get them to do it, yet.
