My grandmother had to do something similar. In order to apply for a "delayed" birth certificate so she could move to America from Canada, she brought in the family bible that recorded her birth, all of her school records, and her mother had to swear to it as well. All this information was actually typed onto the back of her birth certificate as well. This was in Canada, but America may have handled it likewise.
