    2. Hello, I have a "Return of a Birth" for my grandmother, born in Cleveland in 1891. It was submitted to The Board of Health of the City of Cleveland. 1891 is pre-printed on the form. The info requested is: DOB, Sex, Color, Nativity, color & age of each parent, place of child's birth (64 Clara St.), Full name of each parent, Name of medical attendant or midwife & the address. Hand written at the top of the form, likely by a clerk--"Filed in Order". I wish I could make out the midwife's name! Oddly, there is no request for the child's name, which was Amanda Johanna Marie Schultz (Dill). I don't know the source of the copy since this was among Granpa's papers. He probably requested it sometime during the 40's or 50's. So, at least by 1891, some sort of birth record was available though, perhaps, not required. Sorry, I can't offer any more help...
