GO TO THE TOWN OR CITY WHERE THE PERSON DIED AND GET IT FROM THE TOWN OR CITY CLERK.  Most have forms on-line that you can fill out and send in if you live too far away.  As you appear to live in Brooklyn, going in person is the best way--just check the hours the clerk is open first.   Barbara L. de Mare, Esq.
