Sheeesh!!!! Sooooo....I sent away to request a copy of the original application for a Social Security number. And they sent it to me. And (WOW!) is it ever hard to read. I can actually read the information written by hand; but the printed words on the form are almost totally illegible. (No, it's not just that I need new glasses.) Does anyone have one of these that is legible enough that they could scan (or key in, uhhgggg) the form and post it? I would really appreciate it! I THINK I can read most of it -- but in the lower right corner there are some boxes to check and (I think) a blank to fill in next to the top of three(?) boxes. Uhmmmm....what do they say?! This has been a very big Good News/Bad News deal. The only word totally obliterated on this form is the maiden name of the applicant. Sigh. But LOTS of good stuff I really needed and couldn't find anywhere. Well worth the $7.00 (?) it took to get it (after about 10 weeks ... so don't give up!).
