Hi Kellie, Joan Van Loan Registrar of Vital Statistics 1 Garraghan Drive Kingston, NY 12401 Telephone: (914) 331-0080 The Request for Death Transcript form (if going in person) requires the following information; if writing to her, she would need mention of this as well. Name of Deceased: Date of Death: Purpose for which this record is required: I wrote in "genealogy" Number of Copies: Did Deceased die within City Limits of City of Kingston? Yes or No (A couple others I inquired about, I didn't know. I just put in a ? mark. They were not in their records) Died at: (if known) For example: At home, In Hospital, ... What was your relationship to deceased? In What Capacity are you acting? (I left that blank) If Attorney: Name & Relationship of your client to deceased: (Was N/A) Your Signature Your address Date If your relative(s) record is on file there, the fee is $10.00 each, cash if in person, or money order. If they have the record, they type up a "Certified Transcript of Death" which gives name, registered number, date of death, Residence Address, Date of Birth, age, Never Married, Married or Separated, Widowed or Divorced, Spouse (if wife, maiden name) Social Security No., If Veteran, Name of War, Occupation, Birthplace, Father's Name and Place of Birth, Mother's Maiden Name and Place of Birth, Place of Death, Cause of Death, Medical Attendant, Place of Burial or Cremation, Funeral Director, and Informant. Some of this information above, may not have been given or known at the time of death. They stamp it "For Genealogical Research Only" There is a possibility that the Registrar of Vital Statistics in Kingston, may not have records for those who actually died in Lloyd or New Paltz. Those records could be obtained from the individual "towns." In Lloyd: Town Clerk's Office, 12 Church, "Hld," I believe Hld stands for Highland, NY 12528 Telephone: (914) 691-8011 In New Paltz: Town Clerk & Tax Collector, 130 N. Chestnut, New Paltz, NY (914) 255-0100 Some towns charge different fees. Perhaps writing and/or calling first, would be best. As was one case with the Town of Rochester, in Accord, NY, I was told "the fee was $11.00, whether or not they had the actual record!" And as always, the SASE when writing. Good Luck ..
