The original stones are broken but a new stone was put up with all the original information. The park is a part of the Suffolk County Park system from what I gather. It is in an odd location, by today's standards, and I am not sure if you can find it in Flanders, which is a part of Riverhead & Southampton Town, or Hampton Bays, which is Southampton Town. I don't mean to confuse you more. I will gladly drive down and copy the info for you, or take a picture if you'd like me to send it to you. A friend of mine is related to these Hubbards but I am not sure how much he really knows. The Hampton Bays Historical Society may be able to help you.
