The Town of Patchogue? What would the address be and the fee?" For death certificates in Suffolk County you have to contact the village or town where the death occurred. It depends on whether the person died in a village (municipal corporation), and if that village voluntarily keeps vital statistics or if the person died in a village that doesn't keep these records or any of the hamlets (unincorporated areas) in the Town of Brookhaven. In the latter two cases, the Town Clerk for the Town Brookhaven would be the record keeper, while in the former case, the village clerk would be the record keeper. Currently, there are 8 villages and 36 hamlets in the Town of Brookhaven. Patchogue is a village in the Town of Brookhaven, and is currently the location of the town seat of the Town of Brookhaven. But, bear in mind, adding to the difficulty is that the majority of places (individual addresses) in Suffolk County are in a different community than the community that appears as part of that place's mailing address. Here's the contact information for the Town Clerk for the Town of Brookhaven. If the death occurred in a village that keeps such record, the Brookhaven Town Clerk will let you know.
