Lori Ann, It doesn't matter, either as long it was after 1881. The New York State Dept of Health, Vital Records Section, Empire State Plaza, Albany, NY 12237-0023 has the record also. I believe, it's $15 or $21 for a certified copy. I think they have a website where you can download the form..
