Saly I did not know your loved one. It's possible he was transferred to another institution or to a group home. Letchworth Village changed their name to Hudson Valley DDSO many years ago. Many of the builkdings are closed but the medical records office and client information services are still open. the main phone number is (845) 947-6000. You can call them to see if they have records on your loved one. It might take them awhile to find the old records but they will. They are not allowed to disclose any information so they may ask you to send them a request in writing or go there in person in order to ensure client privacy. Let me know how you make out.
