I sent my request for 3 certificates at about the same time, about a month ago, I received a check from the state in the amount of $12.00 referencing the Dept. of Health. Can think of no reason why I got the check or the amount of 12.00, with the exception that they were looking and couldn't find one? Doubt NYS is giving rewards for patience.... Linda
