Hi Kathy, You are very welcome. I don't have the SS-5 form in front of me, so pay whatever they state since you now have the number. That and the additonal address came from two paid membership sites. Since Calvary has the largest number of interred and Joseph was from Queens, I'd lean toward calling them first. Other readers from Rockaway may have other suggestions. Take care, Barb ============= ancestree@verizon.net writes: > Thanks Barb! Thanks Barb! The letter will go out Tuesday requesting the long form for Joseph's SS-5. Other information I found indicates the fee when one knows the number is $27 but you mentioned $29. Are you sure it's $29? The info from ancestry provides the SSN but that's the first mention of Breezy Point. Is Far Rockaway the same as saying Breezy Point? Joseph Brown was Roman Catholic. I'd like to hear suggestions as to where he might have been buried and what Roman Catholic church he might have attended in Breezy Point. From what I've been able to find my guess is St. Genevieve. Thanks! Kathy On Sep 3, 2012, at 3:00 AM, NYQUEENS-request@rootsweb.com wrote: > With this number, you can order the long version of his original > application on the SS-5 form for $29.00. There are directions on the site to order the certificates from the NYC archives. I believe each certificate would be $15 each from the archives. You could make the request Familysearch.org using the form found here - https://familysearch.org/learn/wiki/en/images/5/5f/Request_for_Photocopies.pdf It will be $2 per certificate. (If you haven't used familysearch before let me know and I will look up the michofilm numbers you will need) The certificates should contain birth information - parents names, place. Also the place of death and where they are buried. Important Note: The author of this message may not be subscribed to this list. The McCready's were Irish, so I never would have thought to look at a German genealogy website. Also, now that we have a death certificate number, what do we do with that information?? The surname spelling is off as is the birth year, but the death took place in November in Brooklyn. The death certificate can be ordered from the NYC archives or through the Mormons in Salt Lake City. It may have a cemetery named or it might lead to other things and thus worth a try. Good luck. We went to St. John, New Brunswick this summer and by looking at a book of vital statistics from newspapers, were able to trace the family to Long Island because of an article about Thomas's death that appeared in a St. John newspaper. It stated that his family had lived previously in St. John but were at that time living in Long Island. The librarian did try to look up newspaper articles but I'm not sure of an actual obituary.
