Maybe you might try making a request of one of these two individuals explaining that the reason you are requesting the same information that you had already received in 1996 is because you had lost the information that was sent back then?? The letter that I had sent them back in 2007 asked them to please send my husband any information sheets or forms that they might require us to complete to obtain personal information about his admission, discharge and any other medical information that might pertain to my husband's health. We also stated that when we made a trip to western NY four months later, we could bring them either my husband's birth certificate or his father's death certificate if they might require any of that information to prove my husband's relationship to the mother who admitted him to the home.
