Hi all, I was wondering if anyone could help with information on how I go about applying for a copy of a death certificate for someone who died in 1972... bearing in mind that I'm in New Zealand and cannot go personally to request it. I have been searching for information on my husband's uncle, James McCullough Cunningham for many years. I finally managed to get a copy of some SSDI records which shows the correct James and that he died in Oct 1972.....no day, just month and year. On the day I received his SSDI record I also found his arrival in New York on the SS Transylvania in 1929....think I also managed to find him in the 1930 census. WOW what a great day. Also are there any online (or not) directories/electoral rolls for the 1930 s - 1970's in 1936 when he applied for his SS number he was living at 2032 5th Avenue and was working for Union ****? Bank at 6th Avenue and 40th St. Any help or suggestions appreciated. Cheers, Kate

    3. Hello Kate, I can't answer all your questions, but you can find a form to use by going here:http://vitalrec.com/ny.html#State. Wow... the death certificate there are expensive! I thought when I inquired about one some years back they had to be on file for 75 years. Another suggestion would be to write to a library near where he died. I find addresses through the on-line yellow pages. Sometimes the libraries have the deceased indexed by name, so you might get lucky. Sorry, I can't help you more. I have very few relatives with NY connections.
