Be sure to request the long form [approx. $26.] as it includes more information> name, address, age, name and address of first employer, job title, original signature. I have a family member who is listed on Ancestry in "SS Applications and Claims Index" but this is only an index. Does anyone know if the original documents exist and if they are accessible? Also, to pick your brains a bit: this person seems to be listed twice with two different SS numbers. Same parents, different birth years, though only one year apart. While it could be two different people, was there ever a reason to receive a second, different SS number? Thanks for any help you can offer - Michael
