My mother died this past year, May of 2004 in a Plainview, Northshore Hospital, In Suffolk County close to my brother. I do understand that you have to order them through the town but I haven't been able to find out any addresses and also the amounts needed to send. Can anyone help me? It would be very much appreciated. I do have their death dates so that is not a problem !!!!"
