You can obtain a death certificate if you are the spouse or other blood relative of the deceased, or if you establish your right to obtain this document. For applicants NOT related to the deceased you will need to establish your right to obtain a death certificate by providing any of the following original or certified documents which include both your name and the name of the deceased: Insurance Policy Will Bank Book or statement Property Deed Other document showing entitlement Important Note: The author of this message may not be subscribed to this list.
