You need to tell them the Title and film # of the film which would have the certificate. You find that on the familysearch.org site (in the Library Catalog). In this case, the certificate for Mary should be on the following film: Title: Brooklyn Death certificates, 1919-1949 FHL Film # 2167940 (This film has certs. #11650-13390 4-30 June 1947 ) You can download and print out the form for this request from the familysearch.org site. On the Home Page see where it says "forms, maps and guides" on upper right - click on forms. On the page that appears, scroll down to where it says - "Request for photocopies - census,books, microfilms ..." This is form @ 31768 . After you download and print this out, fill it out with the needed info - with the certificate # being put in all the way on the right (where it says registration no.). It will cost $4.00 (cert. copies are $2 each, minimum $4 per order). Send a Money Order, not a check. It will go much faster that way. It should take 10 - 15 days. I did this a few times last year, so hopefully it's not a longer wait this year. The m/o should be made payabe to the Family and Church History Dept. You must send this order directly to Salt Lake City at the address given on the form. Do not try to place the order at your local FHC, they cannot take your money for this order or process it. Hope I made myself clear, my eye condition is acting up today, but you can check the film # in the Library catalog on the site if you want. Hope this helps you.
