You would have to fill out another NY State form (the same one you did for Arthur), but you need to send it to the City of Buffalo. The site on the NYS Department of Health will give you the address - it goes to the "Local Registrar" in Buffalo. Since you know the year of her death, it would be a simple search. You could put down the various spellings of her maiden name, and the two married names. Did your grandfather ever serve in the armed forces? That would be another source for you, if he did.
