    3. Same happened with my grandfather's death record in Pittsburgh. I was told chances are the mortician wrote in the names. Heck, even his church baptism record in Ireland had his mother's name incorrect. The civil record fortunately had the info correct. I have other records to support the civil record. When my husband's brother died in 1995 in Syracuse NY, his mother's name was missing from the death certificate. About five of us checked the form before it was submitted and no one noticed the error. Should I mention my mother-in-law was in the room with us? Death records should be used as a guide not a fact unless you have backup.
