Good afternoon everyone, This question may have been asked numerous times, but I am looking to obtain the birth certificate of my grandfather, whom was born in NYC in 1914 in Manhattan Borough (the family lived right off of Canal Street). Unfortunately, it appears from the NY Vital Records site, I can only order online if the person ordering the record appears on the birth certificate itself. Since this is my grandfather, this would not be the case. So it seems that one must send in a written (notarized) request, but again, since I would not be named on my grandfather's birth certificate, would my request still be processed. Would appreciate information from anyone that has successfully obtained birth records from the NYC Dept. of Health and Mental Hygiene (since they cover all 5 boroughs for births after 1910) and if I will need to provide any additional documentation to show my relationship. All I have is an old address from where the family was living at the time (not far from the Holland Tunnel and Our Lady of Vilnius church) and I do not know the hospital. Thanks in advance! Richard
