Hi Loreen, I was just going through my e-mails and realized that I never answered you. I'm sorry. What you do is to fill out the request form as if you were requesting the actual certificate. The film number is the one that the index for that year and place would be on. You fill in the name, date, location etc. They look for the index listing, make a copy and send it to you.
