    3. It may be that this death was never reported to the Social Security Administration and therefore the information will not show up in the SSDI. You can still request a copy of the SS-5 but will need to show reasonable need for said document probably. Although I was able to get a copy of my grandfathers SS5 by sending a copy of the death certificate and stating that he was my grandfather on my Dad's side. He also got added to the SSDI after that. "I also found the 'Alease Powell' addition on the SSDI, but the city, and more importantly, the social security number, is not correct from the information that Barb is looking for and that is why I did not send that information to her.
