Hello all, The online DEATH CERTIFICATE APPLICATION at http://www.nyc.gov/html/doh/downloads/pdf/vr/death1.pdf has these statements: "NOTE: The confidential medical report of death, including the cause of death, can only be issued for deaths occurring on or after January 1, 2010." "20. DO YOU NEED THE CAUSE OF DEATH? __YES __NO Cause of Death is only available for deaths occurring on or after January 1, 2010. You are only entitled to the cause of death if your relationship to the decedent is listed below: Please check the appropriate box __ Spouse or Domestic Partner __ Child __ Parent/Legal Guardian __ Person in control of disposition as written on death certificate __ The legal representative of one of the above; please specify" "NOTE: The confidential medical report of death, including the cause of death, can only be issued for deaths occurring on or after January 1, 2010." I wonder why release of cause of death is available only for recent deaths. PJ
