Hello all, I have a certified Certificate of Death (Bureau of Records and Statistics, Department of Health, City of New York) for my grandmother. It was signed on 1949 July 22, for her death in Brooklyn. Above the attending physician's signature is this statement: "I further certify that death + (handwritten 'not') caused, directly or indirectly by accident, homicide, suicide, acute or chronic poisoning, or in any suspicious or unusual manner, and that it was due to NATURAL CAUSES more fully described in the confidential medical report filed with the Department of Health. + See first instruction on reverse of certificate." Questions: 1. Has anyone seen the instructions on the reverse of a death certificate, or know how to obtain them? 2. Has anyone obtained such a "confidential medical report filed with the Department of Health," or know how to obtain one? Thanks for any thoughts. PJ
