PJ My grandfather's fall from the roof top was definitely either a fall or suicide, but he died in the hospital about 7 days later from the extensive injuries received in the fall. The report I received from the coroner was very detailed. I say submit the request, all the coroner's office can do is say no we have no report on this person. Or yes we do.
Barb and "Buffycat," You suggested that I find a coroner's or medical-examiner's report. My grandmother died in a hospital, not at home or elsewhere, so I'm not sure those officials would have been involved. Nevertheless, I'm mailing a request to NYC Dept. of Records for the medical-examiner's report. The application is at http://www.nyc.gov/html/records/downloads/pdf/ocmeform.pdf . Thanks for the suggestion. PJ
Interesting....and curious too. My grandfather died by falling off a rooftop in Manhattan in 1920. I got the full report of cause of death and manner from the coroners office. I guess they must have different rules and regulations than the DOH.

Hi Pat, That property is no longer a Catholic anything; it's private condos now. Mt. Saint Florence was the name given to the whole property owned by the Sisters of the Good Shepherd. This was located on the north side of RT 202, as you enter the City of Peekskill. One section was established to train young women for the novitiate. About 1957, my mother took us there to visit some family friend's daughter, who was new to the convent. Since I read the sign "Villa Loretto---Home for Wayward Girls" (which was posted for many years), and asked my mother what wayward meant. The second section was called Villa Loretto aka The House of the Good Shepherd. It was not a poshy boarding school or "private" academy. Some young women were admitted voluntarily, others were "committed by courts" in the New York metropolitan area, especially from NYC. Someone related to our "married-ins" was sent there from Brooklyn, circa 1945, as her mother felt she was unmanageable. Pregnant girls were also sent there, although some church affiliated folks may deny it. I was unaware of a nearby horse ranch, until some men in their 50s recently spoke of riding horses to Villa Loretto to flirt with the girls. As you may know, in NYS many religious and nonsectarian institutions, psych hospitals, TB sanitariums, cemeteries, orphanages, "homes" and prisons were established along train lines from NYC, especially along the Hudson line of Metro North. It gave easy access for placements and visitors, under the belief then of getting folks out of the city and into the fresh country air. Peekskill was also home to St. Joseph's Home for about 80 yrs, still homebase for the Franciscan sisters, St Peter's Home [Episcopal], and The Peekskill Military Academy [where the NY Jets first practiced]. I hope this answered your curiosity, Barb http://www.amazon.com/Adoption-agencies-orphanages-maternity-homes/dp/0960420037 Page 264 http://www.maltadonna.com/chapel_hill/history.html ==================================================== From: Pat Lewis [email protected] THERE WAS A HOME/ACADEMY? I recommend this article for its history of the death certificate and for its discussion of certificates' limitations. I think online access to it at The New Yorker site is for subscribers only, but maybe the article will eventually be available in various online archives. DEPT. OF PUBLIC HEALTH FINAL FORMS What death certificates can tell us, and what they cant. BY KATHRYN SCHULZ The New Yorker, APRIL 7, 2014. PJ
Hello all, The online DEATH CERTIFICATE APPLICATION at http://www.nyc.gov/html/doh/downloads/pdf/vr/death1.pdf has these statements: "NOTE: The confidential medical report of death, including the cause of death, can only be issued for deaths occurring on or after January 1, 2010." "20. DO YOU NEED THE CAUSE OF DEATH? __YES __NO Cause of Death is only available for deaths occurring on or after January 1, 2010. You are only entitled to the cause of death if your relationship to the decedent is listed below: Please check the appropriate box __ Spouse or Domestic Partner __ Child __ Parent/Legal Guardian __ Person in control of disposition as written on death certificate __ The legal representative of one of the above; please specify" "NOTE: The confidential medical report of death, including the cause of death, can only be issued for deaths occurring on or after January 1, 2010." I wonder why release of cause of death is available only for recent deaths. PJ
The Evening Telegram New York, NY Thursday, January 7, 1897 Page 3, Column 3 THIRTY TIMES IN JAIL Gallagher Was Released From Blackwell's Island Yesterday and Was Sent Back Today. James Gallagher, twenty seven years old, who says he resides at No. 33 West Eighteenth street, was released from the Workhouse on Blackwell's Island, yesterday, having completed a term of six months' imprisonment for assault. This was his twenty-ninth visit to the Island, and today for the thirtieth time In his life. He was arraigned a prisoner In Jefferson Market Police Court on a charge of disorderly conduct. It was Patrolman Spencer of the West Twentieth street station, who arrested him this time, and the hour of the capture was 4:00 o'clock yesterday afternoon. Spencer saw Gallagher and another man quarrelling at the corner of Sixteenth street and Eighth avenue. Gallagher attempted to assault the other man, who ran away. Gallagher hurled a piece of lead pipe, two Inches In diameter and about eight Inches long at him, narrowly missing his head. Spencer grappled with Gallagher and the prison bird showed flight. Spencer rapped for assistance. It took the combined efforts of five policemen to drag Gallagher, shrieking, swearing, fighting and kicking all the way, to the station. In Jefferson Market Police Court, today. Gallagher had no excuse to offer. He sullenly refused to answer any questions and Magistrate Flammer agaln sent him to the Workhouse, where he will again serve a term of six months. transcribed bu Barbb
Hello all, I have a certified Certificate of Death (Bureau of Records and Statistics, Department of Health, City of New York) for my grandmother. It was signed on 1949 July 22, for her death in Brooklyn. Above the attending physician's signature is this statement: "I further certify that death + (handwritten 'not') caused, directly or indirectly by accident, homicide, suicide, acute or chronic poisoning, or in any suspicious or unusual manner, and that it was due to NATURAL CAUSES more fully described in the confidential medical report filed with the Department of Health. + See first instruction on reverse of certificate." Questions: 1. Has anyone seen the instructions on the reverse of a death certificate, or know how to obtain them? 2. Has anyone obtained such a "confidential medical report filed with the Department of Health," or know how to obtain one? Thanks for any thoughts. PJ

Plus there are pages for every civil parish except Cloonclare and Killasnet which will be added in future updates to the site. -- Pat Connors, Sacramento, CA http://www.connorsgenealogy.com
The New York Times published in New York, NY 15 August 1880 GENERAL TELEGRAPH NEWS Immured in A Convent Forty-five Young women take the Black Veil at Baltimore Baltimore, Aug 14.-At the Convent of Notre Dame, at Govanstown, a few miles from this city, this morning 45 young ladies took the black veil. The cars leading to the convent, from an early hour until the ceremonies took place, were crowded with relatives and friends of the young women. Father NEITHEART, of New Orleans, delivered the address, after which the solemn mass of the Holy Ghost was celebrated. The usual ceremonies followed, after which the novices were invested with the black veil. The names of the ladies who took the veil, with their religious names are as follows: [New York only] Miss Ann REEF, of Rochester NY, Sister Mary Bernarda; Miss Mary STAUFF of Rochester NY, Sister Mary Edward; Miss Agnes SMITH of Rochester NY, Sister Mary Gregory; Miss Kate AHERN of New York, Sister Mary Isabel; Miss Kate SCHEEHER of New York, Sister Mary Waltrudis; Miss, Magdelen OSWALD of New York, Sister Mary Gratia; Miss Jennie CORCORAN of New York, Sister Mary Ferdinand; Miss Mary McGANN of Brooklyn, Sister Mary Perpetua; Miss Margaret CARROLL of New York, Sister Mary Mortana; Miss Annie LAUBACHER of New York, Sister Mary Sebalda; Miss Louisa HUBER of New York, Sister Mary Fidelis. transcribed by Barb Source: http://query.nytimes.com/mem/archive-free/pdf?res=9F01E1DC153BE033A25756C1A96E9C94619FD7CF

If you can give me the exact page link, I will try to fix it. I moved my site from one server to another in February and I am sure there are some missing links. Look on the adjacent pages. This won't give you the name if it's a very small street, complete on your page; but it'll still give you clues.
