    2. <PRE>I used to work for the state, and checking birth records was part of my job. Local registrars only provide the name of the child, date and place of birth. For a complete certificate, you must write to the BVS in Trenton. If parents are unmarried, the father must sign an affidavit of paternity in order for the child to have his last name, and for his own name to appear on the certificate. It also possible to have changes made after the original is filed.
