I sent in a request for 14 names and included a blank check. Why? Because I had folks with Retirement that I couldn't find SSN# for. I also had spouses with no SSN. They called me and confirmed the amount and I told them to process the numbers. I did use the Ancestry "form letter" for each request and gave some more detailed info - spouse, etc. on the letter. It took about 2 months to get the forms back. They also included a letter with all the names and SSN# and a TAG number if I needed to contact them again. I didn't find much new information but I was very pleased with the service.
