I have the above book on library loan for the next week and will be glad to research in it for anyone. The church listing is by city. Book contains chronological list of congregations including when founded, who served there. There is also a listing of all ministers in the denomination (including birth place and date, when ordained, their education, where they served, etc.) in the denomination.
