I also had them return my information. I had sent a "blank check" so they had the money! They sent back forms for me to fill out and I also have to send to the Archives section instead of Health and Senior Services! These just came back last week! They also want the name of the municipality in which the event occurred to search for any record prior to 1904. If they died in Newark and were buried in Newark (Woodland or Glendale) won't Newark be OK to use? Does anyone know the municipality?
