Vital records office is in the lower level of the Health and Human Services bldg. which is located in the State office park on Hazen Dr. This is just off Rte. 9 on the Portsmouth Rd. About 2 miles east of Rt. 93 in Concord. There are no signs as to the Vital Records. , but just go in to the HHS bldg. which is Number 6 Hazen Dr. (about a half mile on the left.)
