Fill in lines as follows:- Federal Government Institution -- STATISTICS CANADA NAME, COMPLETE ADDRESS & PHONE NUMBER Mark the little box beside A CANADIAN CITIZEN Sign and date the form -- Allow at least three days when mailed, then 30 days for rejection.
