Cook who died Feb. 18, 1964 in Dayton Certificate # 13895 (if that is helpful). I was going to order a certified copy on-line but it was going to cost me $50.00. Being a great uncle, I was only interested in his birthdate and place, parents names, spouses name, and informant, etc. Thank you so much for any help anyone can give me.
