Marie, Write to the Northwest Missouri Psychiatric Rehabilitation Center the address is: ATT: Health Information Department Northwest Missouri Psychiatric Rehabilitation Center 3505 Frederick Ave St Joseph, Mo 64506 Tell them who you are looking for and the date of their death. They will give you the address to write and get a court order so the information can be released.After receiving this Court Order you then write back to the address above to get the information you are looking for. I am in the proccess of doing the same thing so I can get information on my great grandfather who died at the State County Farm in 1902.
