If you look at the doctor's signature you will see a letter much like the one that starts Harley's last name. Hi there, Rootsweb.com has an entry for William Harley Crouch, same DOB, same DOD, only his place of birth is listed as Il instead of IA. It does have him dying in Cassville, though. Unfortunately, the two entries for him are not openable.
