I don't have the info you asked for but a suggestion. When I find a place I need funeral info on I look in the yellow pages on my computer for funeral homes in that town or county. I then contact them to see if they know of the one I am looking for. This is a round about way but it works. Most listings have addresses and phone numbers. Good Luck!
