When doing Archive death certificates take the time do all version of the surname and do not trust county listings. Many cemeteries have no official name,even not deeded, and many abandoned. So official filling out form didn't put it in or you have an amended certificate. I have not tried lately but sheet one is corticated full size sheet 2 definitions and instruction, sheet three often blank sheet four amendment (sometimes added data) on one occasion I got corner's report too.
