Just received a birth record of my uncle, from Hennepin County Vital Statistics. Note at bottom: "Amended by State Registrar pursuant to affidavit filed on 11-17-(??) 42" I am not sure of the year, but this is what I think it is. How can I find this affidavit? Who to write to? How much does it cost? Susanne
Susanne, I'm not totally sure, but the amended record could have been just a change-- correction, or addition of his name, or something missing from the record and there may not be anything more. This thing happened to me. When I was in my 30's I decided to get a copy of my birth certificate and discovered that it didn't have my name on it, just "Baby Girl" and my last name. So, had to have it amended and have a family member (my sister) sign an affidavit as to my real name. I don't know that there is any other paper work connected to my birth record now showing the change, other than the note at the bottom of my certificate. However, maybe someone else knows more about this procedure?
