All you need to give is the details as per the index entry if you are sure you have the right one. If you're not sure: were the parents married? If not, then the mother's name but don't give the reference, just check the box to say you don't know it. You could also give the fact the child was adopted subsequently. I don't know about Wandsworth Register Office. I usually deal with the GRO but if a register office is unco-operative, the GRO can make them co-operate.
