Jack was born on May 3, 1925 in Bolivar, TN the son of Dave and Annie (Harris) Polk. He is survived by his wife, Rose Sewart Polk, two daughters, Harriett (Ivory Joe) Robinson and Kimela Stewart, 5 grandchildren, a sister, Onetress Robinson of Nazareth, a special Uncle, Herman Stewart of Kalamazoo, an Aunt, Velma Patten, cousins, Donnie and Freddie Jones and a host of nieces and nephews. Funeral Services will be held at 1:00PM on Friday, December 10, 2004 at REDMOND Funeral Home 4100 S. Westnedge Ave. Reverend Henry Dungey officiating. Interment will be in Riverside Cemetery.
