Meyer, but I have not seen this record you posted. Can you tell me what kind of record it is? I'm curious as to what record in Philadelphia archives would contain the name of his personal physician... I want to get his death certificate too but am unsure where he died -- he lived in Vineland, NJ before his death around 1952 but had lived in Philadelphia prior to his marriage. My parents are divorced, so I'm having a hard enough time locating records as is. I did have a family tree once, though, and it does strike a bell.
