Balt, M.D. is the physician who signed the death certificate. His address is 314 N. Exeter St. I have located that address on Mapquest and also located the doctor on the 1900 Census at the address. I am hoping someone can help me with the correct name of the undertaker as well as his address. If he can be accurately identified, I would like to know who has his records.
