If you send me an e-mail I will attach a copy to a return e-mail addressed to you. It says her father is Stanley Strejcek and mother is Catherine Dober Strejcek. The informant was her mother. Birthplace is Easton, Md on Oct 9, 1953. DOD is 8/10/1960 and burial is 8/13/1960 at Mt. Carmel Cemetery. It is signed by a doctor and has a stamp of approval by the medical examiners office. I don't know whether that means an autopsy was performed. The funeral director was William Cook, 1217 St. Paul Street.
