Dear Liz, I received my GGGrandfather's Declaration of Intent and Naturalization papers from a Ms. Elizabeth Bouvier, Judicial Archivist, New Court House, Room 104, Boston, MA 02108. I too had a copy of an index card. My GGGrandfather was naturalized in Nov 1876 and filed his Declaration of Intent in Oct. 1874 with the Worcester Superior Court. I was informed by NARA Waltam, MA that the Massachusetts State Archives would have the original records and that I could get a copy of his file. I emailed the Massachusetts State Archives to inquire about the records and Ms. Jennifer Fauxsmith, Reference Archivist responded. She informed me that these records fall into Ms. Bouvier, Judicial Archivist's jurisdiction and she would be best able to answer any questions and assist me. I emailed Ms. Bouvier and she never emailed me back. Instead within 4 days of emailing her, I had the files in my mailbox!!!!! I was so happy. Unfortunately, I no longer have Ms. Bouvier's email address since my computer crashed. You could try snail mailing her your request at the above address. I tried surfing the net to see if I could find her email, but could not. Good luck with your quest.
