    3. Anthony, You need a Standard Form 180 from the General Services Administration. This is the required "Request pertaining to Military Records". Most libraries have these - call the library in Hamilton to see if they have them. It can be photocopied. I did have the form but I don't think it would scan that well. You would get a DD214 for the Army/Air Force in WWII and I forget the ID for the Navy. If the library doesn't have the Form 180 let me know and I will find MY INFO and you can get it.
